Children’s Community Nursing Team
We provide support to parents and carers of children and young people who require nursing care at home.
The team provides a service for babies, children and young adults 0-18 (up to their 19th birthday) with a wide range of nursing need, who are registered with a Leeds GP, or live within the Leeds geographical boundary, but are not registered with a GP.
The team provides advice and support seven days a week with a reduced service at weekends and Bank Holidays.
What do we do?
- We provide nursing care and treatment plans, support, advice, knowledge and skills to empower patients and families. This may be for example wound care, pin site care, gastrostomy, naso-gastric tube insertion, weight management, central line cares, bloods, and community intravenous antibiotics.
- We provide end of life care to enable children and their families to achieve their wishes at a difficult time
- We work closely with the multi disciplinary team and external agencies, with an aim to providing holistic care for the children and their families.
- We provide teaching to all agencies involved in a child’s care. This may be to parents, schools and nurseries and social care.
Referral
We take referrals from GPs and paediatricians from Leeds Teaching Hospitals NHS Trust and other local hospitals.
